How to understand your fertility test results
Fertility tests measure specific pieces of machinery: how many eggs sit in reserve, whether one is released each month, and how well sperm are being made. None of them measures the thing you actually want to know, which is your chance of conceiving naturally.
That gap between what a test measures and what a result gets treated as is where most of the worry starts. This guide walks through each test on a standard Australian work-up, what the number means and where its limits are. When is fertility testing recommended in Australia?
Fertility testing is recommended after 12 months of trying if you are under 35, and after six months of trying if you are 35 or older.¹˒²
That is the formal definition of infertility, and it is the point where RANZCOG recommends women over 35 be offered referral to a fertility specialist.¹ Around 15% of couples will meet it, so being in this room is common rather than rare.²
Testing earlier is appropriate when something in your history already raises the question. That includes irregular or absent periods, known or suspected endometriosis, tubal or uterine problems, previous chemotherapy or radiotherapy, the loss of an ovary, or a known or suspected sperm issue.²
Your GP can order the entire initial work-up, for both of you, without a specialist referral. The tests below are the standard first round, and understanding them before the follow-up appointment puts you in a much stronger position to ask good questions.
The first thing to know: what no fertility test can tell you
No standard fertility test predicts your chance of a natural pregnancy, and no test anywhere measures egg quality directly.
The best evidence for the first half of that sentence is a prospective cohort study of 981 women aged 30 to 44 with no history of infertility, published in JAMA. Women with low AMH or high FSH were no less likely to conceive over six or 12 cycles of trying than women with normal values.³ As a cohort study it describes association rather than cause, and what it found was no association at all between these markers and conceiving.
Egg quality, the other thing everyone wants measured, has no direct test. AMH and antral follicle count estimate how many eggs remain, not how good they are, and age remains the strongest determinant of quality.² We unpack both of these properly in AMH is not a fertility score and CoQ10 and egg quality.
So read every result below as an answer to its own narrow question, not as a verdict on you.
What each fertility test result actually means
Each test answers one question well, and the trouble starts when a result is stretched beyond it.
What does an AMH test actually tell you?
AMH reflects the size of your remaining egg supply, not the quality of those eggs and not your odds of conceiving.
The hormone is made by the small follicles in your ovaries, so the level tracks how many are present. It barely moves across the menstrual cycle, which is why it can be tested on any day, and a good report gives your result as a centile for your age rather than a raw number alone.²
What AMH genuinely predicts is your response to ovarian stimulation in IVF: a low result predicts fewer eggs collected, a high result predicts an excessive response that needs careful dosing.² For natural conception, it correlates poorly, which is the polite way the research literature says it is not a fertility score.²˒³
One Australian practicality: AMH is generally not Medicare rebated when it is ordered simply to see where you stand, so ask about the out of pocket cost when the test is requested.⁴
Why is my AMH lower on the pill?
Hormonal contraception suppresses AMH, so a result taken on the pill reads lower than your true baseline.
In a population study of 42,684 women, AMH values in combined pill users were around 17% lower on average than in women cycling naturally, with lower values also seen in users of the implant, the vaginal ring and the progestogen-only pill.⁵ The suppression looked largest in women whose AMH was already low, exactly the group most likely to be alarmed by their result.⁵
Two calibration notes. This was a cross-sectional study using data from a company that sells hormone tests, which is disclosed in the paper, so it shows patterns rather than proof of mechanism. And duration of pill use was not associated with lower AMH, which fits suppression rather than damage: the pill is not using up your eggs.⁵
If your AMH was tested on contraception and the result matters to a decision, talk to your GP about whether and when to retest.
What do day 2 to 4 FSH and oestradiol results mean?
A high FSH at the start of your cycle suggests your brain is having to shout louder to get a follicle growing, and it only makes sense read alongside oestradiol.
FSH is the pituitary hormone that recruits a follicle each cycle. When ovarian reserve is low, the usual feedback quietens and FSH rises, which is why it is drawn early in the cycle, on day 2 to 4.² An FSH above 10 IU/L is quite specific for a poor response to IVF stimulation, but it misses many women who respond poorly, and it is a weaker predictor again for conception itself. It also swings noticeably from cycle to cycle, so a single high value is not a verdict.²
Oestradiol is taken at the same time because a high oestradiol can push FSH back into the normal range and hide the signal. A normal FSH with a high oestradiol is therefore not the reassurance it appears to be, which is exactly why the two are ordered together.²
How does a progesterone test confirm ovulation?
A rise in progesterone in the week before your period confirms you ovulated that cycle, and the timing of the blood draw is the whole game.
If your cycles arrive regularly, every 21 to 35 days with a consistent pattern, that history alone indicates you are ovulating and a progesterone test is not routinely needed.² Where there is genuine uncertainty, the test is confirmatory, provided it is taken in the mid-luteal phase, meaning seven days before your next expected period.²
This is why the common label of a day 21 progesterone misleads. Day 21 is only mid-luteal if your cycle is 28 days. On a 35 day cycle the right day is 28, and the result is interpreted once your period actually arrives, confirming the timing was right.² Laboratories differ in the exact level they take as evidence of ovulation, so let your GP interpret the number rather than the reference range on the report.

How do you read a semen analysis?
The reference limits on a semen analysis are not pass marks. They are the 5th centile of men who recently fathered a child.
The current WHO reference limits, from the sixth edition of its laboratory manual, are a concentration of 16 million sperm per millilitre, 42% total motility, 30% progressive motility and 4% normally shaped forms.⁶ Those numbers describe the bottom of the range found in about 3,500 men whose partners conceived within a year, which means 5% of demonstrably fertile men sat below them.⁶
So a result below a reference limit does not mean infertile, and a result above them does not guarantee the opposite. Semen results also vary meaningfully between samples from the same man, so an unexpected result is usually repeated before any conclusion is drawn.⁶
The practical point is simpler: the sperm half of the equation gets tested late or not at all in many couples, and it is a single non-invasive test. We cover what actually moves these numbers in sperm health and paternal preconception.

What else is in a standard fertility work-up?
Alongside the headline tests, a standard Australian work-up includes thyroid function, a pelvic ultrasound and the routine preconception screen.
The blood panel usually adds TSH, and where cycles are irregular your GP will add prolactin and androgen testing to look for the cause rather than just confirming the fact.² A transvaginal ultrasound counts antral follicles and checks the uterus and ovaries for polyps, fibroids and signs of endometriosis, which blood tests cannot see.²
The preconception basics travel with all of this: rubella and varicella immunity, blood group, a full blood count and the option of genetic carrier screening.² If immunity is missing, sorting it early matters because of the live vaccine window before pregnancy, and it is worth having your ferritin checked before pregnancy on the same form.

What this means for you
Ask for a copy of every result, and make sure you have the numbers rather than whether things came back normal.
For AMH, ask for the centile for your age. For FSH, ask whether oestradiol was drawn at the same time. For progesterone, check the test was timed to your cycle length, not to day 21 by default. And if you are a couple, book the semen analysis in the same round as the bloods rather than months later.
If you are 35 or older and 6 months in, ask directly about referral, because that is the guideline position, not pushiness.¹ And if a result worries you, bring it back to your GP with one question: what does this change about our plan?
